Enrollment Forms

Please read, print, and sign all forms in correspondence with your child’s enrollment at New Horizon. All forms are REQUIRED to be turned in before your child is authorized to enter the facility.

Copies are available for pick up at every facility

  • Please read, print, fill out, and submit this Emergency Contact Form to your child’s learning center before their start date.

  • Please read, print, fill out, and submit this Transportation Authorization Form to your child’s learning center before their start date.

  • Please have a licensed physician fill out this Child Health Report and return it to your child’s learning facility before their start date.
